1. Must complete COVID-pre screening form prior to every treatment and Health History Form (new clients only). This is sent out with reminder email 3 days prior and can be filled out online. If you have any symptoms, we ask that you reschedule your appointment to atleast 10 days later.
2. Please DO NOT arrive early. If you are early, please wait in your car.
3. Please remove shoes upon entry and sanitize hands. Clean slippers are provided or you can bring your own.
4. There will NOT be any blankets provided during this time, please bring your own if desired. You will still be fully covered with sheets, we will only undrape the areas we are working on.
5. All invoices will be emailed at the end of the treatment.
6. Strict 72hr cancellation policy. Clients will be charged 75% of the treatment fee for late cancellations and 100% for missed appointments. Fee must be paid before rescheduling. If you are late, the appointment will still end at the same time with no change in fee. If you do not show up within 15 mins, your appointment will automically be forfeited.
